A phase 1 study evaluated anitocabtagene autoleucel (anito-cel), a BCMA-directed CAR T-cell therapy using a novel d-domain binder, in 38 heavily pretreated patients with relapsed or refractory multiple myeloma. The trial reported that all patients responded, with 79% achieving complete response, median progression-free survival of 30.2 months, and 65% overall survival at 36 months; grade 3 or higher cytokine release syndrome and neurotoxicity were rare. In vitro comparisons cited in the study found the ddBCMA binder had a faster off-rate than a dual-VHH binder used in another approved CAR T product, with less cytokine release and no off-target cytotoxicity.
